Twenty-eight states will receive $36 million in grant programs to help older Americans, veterans and people with Alzheimer’s receive home- and community-based care, the Department of Health and Human Services said Monday.

The grant program is a joint effort between HHS and the Department of Veterans Affairs. It is part of a Nursing Home Diversion grants program, HHS said in a news release.

“This unique effort supports the President’s New Freedom Initiative which calls upon all federal agencies to help people who need long-term care and prefer to live in their own homes and communities to do so,” said Michael Leavitt, secretary of HHS.
