Healthcare technology firm WellSky has teamed up with Vytalize Health, an accountable care organization, to help enhance physician-to-physician communication and collaboration for better patient experiences and improved value-based care outcomes.

WellSky serves more than 20,000 sites, including post-acute facilities, government agencies, human services organizations and some of the largest US hospital systems. The collaboration will allow Vytalize to connect with WellSky’s network of acute and post-acute care providers and allow for more effective communication.

Acute and post-acute firms receive a notice when a Vytalize patient enters their care. Then, at the time of discharge, WellSky helps providers refer their patient to the most effective next level of care, the
company explained in a statement.

“When providers across the continuum work together to deliver coordinated high-quality care, the result is healthier, more engaged patients,” said Vytalize Chief Product Officer Frank Bordonaro.

More collaboration, WellSky leaders said, can reduce care gaps, eliminate redundant services, reduce readmissions and provide better transparency into patients’ health. 

“We know that ambulatory providers want to be more engaged with their patients’ care plans when something critical happens and collaborate with providers across acute and post-acute settings,” Lissy Hu, MD, WellSky’s president of connected networks, said in a statement. “We have built solutions and services that give Vytalize Health’s partner providers the information they need to take directed action and succeed in value-based care — all while continuing to put patient care first.”
