The American Society of Consultant Pharmacists is hosting a two-day forum for post-acute care pharmacy stakeholders Monday and Tuesday in Baltimore. The first-of-its kind conference will gather industry experts to delve into pharmaceutical care options for providers throughout the continuum of care.

Presenters will discuss how pharmaceutical expertise can positively affect census as well as decrease readmissions and rehospitalizations following discharge, thereby improving the bottom line.

“The landscape of our industry is shifting,” said ASCP Executive Director and CEO Frank Grosso, RPh, ASCP, who will moderate the event. “We hear daily how successful care transitions result in improved patient outcomes and minimize additional expense to the whole health care system.”
