TeamACO became the newest accountable care organization specifically dedicated to meeting the healthcare needs of post-acute care patients. 

The Knoxville, TN-based TeamHealth physicians group announced the launch Feb. 2. The ACO will work with providers and residents in the long-term care space, Josh Hopson, TeamHealth vice president of integrated marketing communications, told McKnight’s.

TeamHealth is already well known in the sector for providing advanced practice clinical support, telehealth and specialty care services. The Centers for Medicare & Medicaid Services approved TeamACO in January as part of the Medicare Shared Savings Program. TeamACO’s focus is collaborative care coordination in post-acute care.

Moving forward, TeamACO said it will focus on collaborative care coordination through advance care planning, reduced readmissions, and gradual dose reductions “to optimize long-term health outcomes for thousands of Medicare enrollees in more than 30 states.”

“A pioneer in the post-acute care setting, TeamACO aligns the rewards and incentive of all who contribute to quality care and assists Medicare beneficiaries in getting the care they need in the most appropriate setting,” said Darren Swenson, MD, president of TeamHealth Post-Acute Care and TeamACO. “The addition of TeamACO to the framework of current post-acute care services will enhance the success of clinicians in post-acute facilities nationwide.
