Signature HealthCARE of Memphis was sold for $13 million at the end of September.
OHI Asset, an affiliate of Omega Healthcare Investors that owns 21 facilities in Tennessee, purchased the property from HCRI Tennessee Properties LLC.
The 140-bed facility was built in 1983.
