Three firms announced they will merge to form one of the nation’s largest home care companies.

Dallas-based Jordan Health Services, which includes skilled nursing and hospice in its portfolio, is joining up with both Great Lakes Caring in Michigan and National Home Health Care, in Connecticut. Together, these three companies will combine to serve some 63,000 patients daily, with 31,000 employees, across 15 states. All told, they’ll have 221 locations. The name of the new company is yet to be determined.

G. Scott Herman, now the head of National Home Health Care in Cromwell, CT, will serve as CEO.

“The combined companies bring together best in class technology, market leading quality and a differentiated care model that clearly sets us apart,” Herman said in an announcement. “As a combined entity, we are exceptionally well-positioned for long term success, particularly as healthcare moves toward population-based health management and outcomes-based performance.”
