PointRight has created a new mobile app called PR Pronto!™. Providers will be better able to make data-driven decisions for patients transferring between acute to postacute care, the company said.

PR Pronto! includes PointRight® Pro 30™, which allows providers to ascertain rehospitalization risk. The app lets SNF intake coordinators and pre-admissions staff screen potential residents and looks at rehospitalization risk upon admission. This eliminates the need to rely on the first MDS, the company said.

“Post-acute providers need tools that enable them to make accurate risk assessments, such as risk of rehospitalization, at the onset of admission so they can make the right care decisions for the patient,” said Steven Littlehale, executive vice president and chief clinical officer at PointRight. “Providers will now understand precisely what the rehospitalization risk is ahead of admission through this analytics-powered screening tool driven by the PointRight Pro 30 Rehospitalization measure, with more analytics coming in the future.”

The company plans to augment PR Pronto! with more predictive models and analytics in the near future, including the PointRight mortality risk index, in the next edition of the app.
