naviHealth and Cigna-HealthSpring have announced a collaboration in managing post-acute care services for approximately 100,000 Cigna-HealthSpring members in the Mid-Atlantic and Pennsylvania markets.
naviHealth’s decision support technology and field-based care managers will be used for Cigna-HealthSpring members receiving skilled nursing facility services, the company said. The program means naviHealth’s nurses and therapists will work with Cigna-HealthSpring members in the hospital for options related to post-acute care and rehabilitation.
After discharge, naviHealth will continue to work with patients in SNFs. The goal is to coordinate care and improve outcomes, the company said. naviHealth partners with health plans and healthcare providers to manage post-acute care.
