A provider of home-based healthcare services has found a technology partner, hoping to give skilled nursing and other providers a window into their patients’ lives following discharge.

PreparedHealth — which is based in Chicago and provides tools to help caregivers across the spectrum communicate — has announced that it is teaming up with Homewatch Caregivers. The latter is based in Greenwood Village, CO, and provides home-based services, including eldercare and assistance for those with dementia.

The home-based care provider, which has some 190 locations in seven countries, will begin using PreparedHealth’s “enTouch” a tech tool that allows providers from all walks to communicate and stay connected. It aims to provide greater visibility into a patient’s care for skilled nursing and other providers, according to a company announcement. The Chicago company’s aim, it says, is to speed up transition times from one setting to the next, while helping SNFs to limit hospital readmissions.

With more emphasis placed on home care by Medicare, we’re excited to provide innovative tools that can enhance patient care and allow us to build stronger relationships with our partners across the care continuum,” Julie Smith, president and CEO of Homewatch, said in the announcement.
