Michigan’s Great Lakes Health Connect has partnered with VorroHealth to install a clinical interface BridgeGateHealth™ in its skilled nursing facilities and home health agencies.

BridgeGateHealth extracts clinically relevant patient information from the Minimum Data Set and OASIS (Outcome and Assessment Information Set) and converts it into a standardized Continuity-of-Care Document.

GLHC’s Virtual Integrated Patient Record solution allows providers a comprehensive long view of a health record and treatment.
