Collective Medical and PointClickCare Technologies have begun a partnership to integrate Collective’s solution for care transitions with PointClickCare’s cloud-based software platform.
The partnership allows post-acute providers that use PointClickCare’s platform to coordinate care transitions and reduce readmissions.
