Leading health, aging services and consumer advocates will meet next Friday (Dec. 10) in Washington to examine the development of "person-centered" quality measures for the elderly and frail. The first summit meeting of the Long-Term Quality Alliance aims to capitalize on opportunities provided through the Affordable Care Act.

LTQA's goal is to act as a rallying point for innovative caregivers. The group hopes to help create relationships, networks and services that will lead to improved care transitions, fewer avoidable hospitalizations and better integrated services for those who receive long-term services and supports.
