Total Living Choices, a Seattle-based company, is now Ensocare. The company focuses on transitions between hospitals and post-acute providers, offering software that can track patients and provide referrals.
Post-acute care providers can participate in electronic referrals for free and create a profile with Ensocare.
Wayne Sensor began as the CEO at the company last month. Mike Cassling previously held the position. Cassling will continue to be the president and CEO of parent company CQuence Health Group.
