Lorien Health Services recently signed with Nexus Health Resources to start using NexuConnexions software in nine of its skilled nursing facilities in Maryland.

The software allows clinical and non-clinical staff to create a tailored agenda for each patient based on individual diagnosis, risk factors and care setting, the company said. NexusConnexions will allow Lorien staff to manage every interaction between patients and caregivers and enhance positive outcomes, as well as reduce readmissions.

“In an ongoing effort to keep pace with the rapid changes we face in healthcare today, Lorien is constantly looking for programs that can help our caregivers be more efficient and productive regardless of setting, whether it be in our nursing centers, in patients’ private residences following discharge through our Lorien@Home program, or to residents of our assisted living communities.” said Lorien CEO Lou Grimmel Sr.

Reports compare actual and target goals for readmissions, network utilization and total spend, the company said.
