Brookside Health and Rehabilitation Center in White River Junction told residents last week that it would close at the end of November. The announcement followed the Centers for Medicare & Medicaid Services decertifying the agency and stopping payments, according to local reports.

The 67-bed facility has received deficiencies in state surveys for food safety, staffing levels and other requirements of participation. One report said staff members failed to supervise and assist 13 of the facility’s 39 patients.
