PointRight Inc. has released PointRight® Pro 30™, an upgraded version of its tools to track and to reduce rehospitalization rates in long-term care facilities. The product was previously called OnPoint 30.

Pro 30 adds facility-level, case-mix adjusted rehospitalization rates for diagnostic sub-categories and for high- , medium- and low-risk groups — capabilities previously only available as a separate product. Customers can now evaluate rehospitalization rates for common issues including congestive heart failure, COPD, stroke, diabetes, pneumonia and surgery.

Pro 30 gives clients access to observed and case-mix adjusted rehospitalization rates for both Medicare and all-payer groups, and compares these with national benchmarks.

“Reducing rehospitalization has become one of the most important competitive factors for providers today,” said Steven Littlehale, executive vice president and chief clinical officer at PointRight. “Acute-care partners and other referral sources are demanding hard data on this quality measure, which, in turn, weighs heavily in decisions for contracts and referrals.”
