Netsmart, a post-acute technology provider, just announced plans to broaden its reach into home and hospice care.

The Overland Park, KS-based tech company will do so by acquiring the home care and hospice offerings of Change Healthcare, a technology provider based in Nashville, TN. In an announcement, Netsmart said the move to acquire these divisions, formerly known as McKesson Homecare and McKesson Hospice, will help establish it as leader in post-acute tech.

“With the Change Healthcare homecare and hospice team and their solutions, we are adding deep domain knowledge to our team, which is key for us to continue to move at the speed in which healthcare is consistently changing,” Netsmart CEO Mike Valentine said in the announcement.

Terms of the transaction, which is expected to close in the second quarter of 2018, were not disclosed. Netsmart said it plans to integrate Change Healthcare’s offerings into its own technology. The Netsmart platform helps connect post-acute providers, such as behavioral health and senior living, with their health system and primary care referral partners. To date, it says it has made more than 1 million such connections, representing more than 10 million clinical transactions each year.
