Home care provider, tech company provide SNFs with window into patients' lives

A provider of home-based healthcare services has found a technology partner, hoping to give skilled nursing and other providers a window into their patients' lives following discharge.

PreparedHealth — which is based in Chicago and provides tools to help caregivers across the spectrum communicate — has announced that it is teaming up with Homewatch Caregivers. The latter is based in Greenwood Village, CO, and provides home-based services, including eldercare and assistance for those with dementia.

The home-based care provider, which has some 190 locations in seven countries, will begin using PreparedHealth's “enTouch” a tech tool that allows providers from all walks to communicate and stay connected. It aims to provide greater visibility into a patient's care for skilled nursing and other providers, according to a company announcement. The Chicago company's aim, it says, is to speed up transition times from one setting to the next, while helping SNFs to limit hospital readmissions.
