Intel-GE Care Innovations™ announced it is working with Front Porch communities and the Front Porch Center for Innovation and Wellbeing to measure remote patient monitoring among senior living residents.

Residents of Kingsley Manor in Los Angeles and Claremont Manor in Claremont, CA, will use Health Harmony, Care Innovations’ RPM platform designed for home health. Health Harmony will be available to certain Front Porch residents who may need more care or monitoring due to chronic conditions, or following a return to a community after leaving a skilled nursing facility. There will be daily health sessions to capture vital signs such as blood pressure, weight, glucose levels or other information.

“Health Harmony gives Front Porch a way to extend the care continuum directly to their residents while also supporting the independence they strive to deliver every day,” said Sean Slovenski, CEO of Care Innovations.

Residents also can use a video chat platform through Health Harmony that enables a caregiver and the resident to discuss a resident’s environment or health.
