Since scooping up a Transitions Gold Award in McKnight’s 2015 technology awards, officials at Colonial Park Care Center say their use of the InTouch Telehealth Network has continued to boost residents’ clinical outcomes and reduce readmissions.

“The continuity of care with our residents has reached a new level with the telehealth system,” says Brett Shibley, administrator at the Harrisburg, PA, facility. 

Colonial Park also has moved beyond using the system primarily for meet-and-greets between new residents and a physician.

“We are now using it for changes in condition and falls to evaluate if the resident needs to be sent to the hospital,” says clinical coordinator Uma Maddasani.

 Up next is an in-house EKG machine that will be read using telehealth, Colonial Park officials say.
