EmpRes Healthcare has selected Nexus Health Resources for its transitional care program, using NexusConnexions and the Nexus Health Call Center.
The company’s transitional care software allows EmpRes staff to oversee day-to-day activities and set an agenda for each patient based on diagnosis, risk factors and care setting.
EmpRes will also use the Nexus Health Call Center for automated follow-up calls after discharge, the company said.
