Relias has released the Relias Population Health Solution, a cloud-based HIPAA Compliant Population Health Platform.
The system has industry standard and risk-based analyses through the consumer, population and provider level. It was designed with a set of more than 200 evidence-based and standards-based measures.
Providers can see information for improvement, including risk stratification tied to performance measures, Relias said.
“The Relias Population Health Solution goes beyond simply identifying care gaps to stratifying risk and providing evidence-based treatment clinical considerations that are recommended to achieving optimal care,” said Carol Clayton, Ph.D., Executive Vice President of Population Health at Relias. “These care considerations can be used by care management and care coordinator teams to direct or inform best care actions.”
