Redstone Presbyterian SeniorCare in Greensburg, PA, has begun using the NexusConnexions® transitional care software at its Redstone Highlands continuing care retirement community.
The CCRC plans to offer the software solution to more efficiently track and manage post-discharge activities.

Patient and caregiver interactions are tracked via NexusConnexions reports with a goal of preventing readmissions.
