The Post-Acute Care Network, a joint partnership between Life Enriching Communities and Episcopal Retirement Services, is changing its name to Concentric Health

Company officials said the new name more accurately reflects an evolution since the business’ 2011 founding by depicting a mission of driving successful outcomes and decreasing costs of post-acute care. 

The circle of the new logo represents the various collaborations between care providers and patients.

Concentric Health assists its members by redesigning care delivery models, implementing care transition programs, and analyzing metrics to assure that patients receive better care at a lower cost in the post-acute care setting. The Concentric Health Network includes 11 provider partners located across Cincinnati, Columbus, Dayton and Northern Kentucky.

“We made it our purpose to change with intention,” said CEO Tim Grimes. “In doing so, we not only developed the new brand and logo, we refined our mission, vision and values. … Post-acute care should be designed to positively impact lives by putting the patient at the center of all that we do. We started our company with that as our vision, and it remains true today.”

Based on Medicare claims data for 30-day episodes of care in the Cincinnati market, Concentric Health said its partners’ average skilled nursing facility length of stay is nearly eight days lower than market; total cost of care after hospital discharge is $3,000 lower than market; and rate of readmission during the 30 days after hospital discharge is 19% lower than market.
