Hanover Hill chooses post-discharge system

Hanover Hill Health Care Center in Manchester, NH, has selected Nexus Health Resources for managing post-discharge patient follow-up, through the use of the NexusConnexions software.

The NexusConnexions transitional care software will allow Hanover Hill staff to follow each patient based on individual diagnosis. Caregivers can track patients with reports, compare goals and target preventing readmissions.
