Six months after starting to use NexusConnexions® and the Nexus Health Call Center® for its transitional care program, EmpRes will introduce Nexus Health Resources software and services to all of its facilities, the company announced.

The expanded partnership will add Nexus Health Academy to 44 facilities.

“We have seen tremendous success in the last six months since we implemented the use of NexusConnexions and the Nexus Health Call Center in select facilities,” said Deb Sanderfield-Hoven, Vice President of Network Innovations for EmpRes. “The decision to expand the partnership to 44 facilities and add the use of Nexus Health Academy was a natural evolution.”

Empres staff can better manage activities of a transitional care team with a tailored agenda for each patient based on individual diagnosis, risk factors, and care setting, company executives explained. Patient and caregiver interactions are tracked using NexusConnexions reports. By adding the academy options, patients and caregivers can receive more education upon discharge.
