Caremerge has created the Chronic Care Management Solution, which can help connect the fragmented aspects of chronic care coordination.
The Caremerge solution will let physicians streamline care coordination, the company said. It will help eliminate the need to pay outside providers, while maximizing Medicare reimbursement.
The company said there are no other technology platforms that offer its level of connectivity to physicians dealing with the chronically ill. CCM “is a natural extension of the Caremerge coordination network,” the company said.
