Measure offers chronic care relief
A new bill would link eligible seniors suffering from chronic illnesses with a health professional to coordinate their care.Sen. Blanche Lincoln (D-AR) introduced the bill this week in conjunction with a Senate Special Committee on Aging hearing on Medicare and chronic care coordination. Lincoln has introduced the bill each year since 2001.
The "Geriatric Assessment and Chronic Care Coordination Act" would allow individuals who have been assessed and deemed eligible for care coordination services to choose a chronic care manager.
