The long-term care community should adopt a standardized patient transfer/summary of care document to coordinate care across settings, according to a plan recently released by a coalition of long-term care and information technology stakeholders.

"A Road Map for Health IT in Long Term Care" is the result of a summit that convened in August in Chicago to build consensus on health IT priorities in the long-term care field. The plan includes recommendations and action points for providers.

Other specific action items mentioned in the plan include prioritizing e-prescribing and medication safety initiatives for the long-term care community; and the certification of electronic health records and e-prescribing solutions.

The coalition included provider organizations, IT developers, government agencies and consumer-group representatives.
