HCR ManorCare enlists PAC company for management

naviHealth, a post-acute care management company, has announced a partnership with HCR ManorCare.

The large skilled nursing facility chain, which has more than 500 centers, will use naviHealth's “decision support technology, post-hospital (post-acute) analytics platform and network of clinicians,” the companies said. 

“We believe that this relationship with naviHealth will positively impact patients' outcomes and goals. Working with naviHealth is only one example of the wider shift towards a more value-focused approach to the way post-acute care will be managed and delivered nationally,” said HCR ManorCare Executive Vice President and Chief Operating Officer Steve Cavanaugh.

Clay Richards, naviHealth president and CEO, said the partnership would improve outcomes for seniors who need post-acute care.  

“We are teaming up with HCR ManorCare to bring comprehensive care management services to its eligible skilled nursing facility providers to further our aim to improve delivery and management of post-acute care across the country,” he said. 

The company announced a similar partnership with long-term care chain Golden Living in December. Both partnerships mean the skilled nursing providers will be included in naviHealth proprietary provider networks. 

It was one of many recent accomplishments for naviHealth, which has headquarters in Nashville and oversees PAC management services to more than a million Medicare beneficiaries. The firm is in 15 states and recently expanded its office footprint.

The organization also was selected as a convener for the Bundled Payment for Care Improvement Initiative, Model 2, through the Center for Medicare and Medicaid Innovation. It also launched its Post-Acute Care Center for Research in mid-2014.
