A new initiative called Aging with Dignity will result in the nationwide distribution of more than 500,000 advance directives over the coming year.

The U.S. Administration on Aging, United Health Foundation, the American Hospital Association and other national and local provider groups support the “Five Wishes” advance directive, which is printed in 20 different languages. The directive will go to individuals, community organizations, hospitals and hospices.

The document is an easy-to-use advance directive that helps individuals and families address their medical, personal, emotional and spiritual needs and wishes before a health crisis arises. The document allows people to spell out type of medical treatment desired, comfort measures to be taken and other critical information for loved ones.
