Post-acute providers must increasingly partner with local hospitals to effectively manage patients throughout the continuum of care. In “Partnering with Your Local Hospitals to Manage Patients Without Re-hospitalizations,” participants will hear from a medical director at a skilled nursing facility to learn how they are building relationships with their area hospitals to be the provider of choice. 

Topics will include how post-acute facilities can work with their area hospitals to gain more referrals and how technology can help improve clinical outcomes.

The webinar will begin at 1 p.m. on Thursday, Feb. 22. The featured speaker will be Zachary Palace, M.D., medical director at Hebrew Home of Riverdale. The session will be moderated by McKnight’s Senior Editor Elizabeth Newman.
