Interdisciplinary Collaboration: How to Achieve it in a PDPM Era

The Skilled Nursing industry is facing implementation of the broadest payment reform in the past 20 years in the form of the Patient Driven Payment Model (PDPM). Much of the industry-wide discussion around the transition to PDPM centers on the need for interdisciplinary collaboration – improved communication, patient-centered care, and alignment of outcomes. Operationalizing collaboration requires the development and implementation of processes and systems to identify key areas of opportunity for collaboration and ensure consistency and focus. This webinar will examine three such areas of opportunity – MDS Coding and Completion, Discharge Planning, and Restorative Nursing Programs.
At the conclusion of this training, participants will be able to:
Execute collaborative processes to ensure key sections of the MDS are completed accurately
Operationalize an enhanced care transitions program for smooth and effective patient hand-offs
Implement effective Restorative Nursing Programming to enhance patient outcomes
