Philadelphia-based medical technology company RightCare has announced a collaboration with Emmi Solutions in an effort to help providers reduce readmission rates.

RightCare will offer Emmi Solutions’ technology with its comprehensive end-to-end solution, adding patient engagement and post-discharge outreach capabilities.  RightCare’s software —  RightCare Risk Assessment, RightCare Coordination Central, and RightCare Post Care Connect — are designed to assess patient risk and needs at the time of admission, ensure the most appropriate post-acute care plan is offered, and seamlessly transition patient information to the post-acute care providers. The new product will be marketed as RightCall.

“Emmi Solutions’ interactive call campaigns enable RightCare to take patient engagement to the next level. Now we can monitor their recovery and anticipate red flags that could result in a readmission,” said Eric Heil, co-founder, president and CEO of RightCare.

RightCare also announced Tuesday it had secured $4 million in additional capital from NewSpring Health Capital. NewSpring General Partner Brian Murphy will join RightCare’s Board of Directors.
