Facility selects dementia care navigation company
The New Jewish Home has partnered with ReServe, a nonprofit, with the goal of matching those older than 55 to provide dementia care navigation services to rehabilitation patients after discharge.
The navigators will be trained by ReServe and assigned to Jewish Home rehabilitation patients with dementia. Each patient or family member will receive a check-in call at least once a week after discharge for three months, plus two home visits. Three navigators have completed training and started working with patients.
“With people living longer, and more and more of us opting to ‘age in place' at home, the need for healthcare professionals who can connect patients to home- and community-based services has become greater than ever before,” said Audrey Weiner, president and CEO of The New Jewish Home. “With their deep well of life and professional experience, the members of ReServe are ideally suited to this role.”
Navigators connect families to resources, plus help learn of services such as home health aides, telemedicine or transportation options.
