Coccomo Memorial in Meriden, CT; Hewitt Health & Rehabilitation Center in Shelton, CT; Plainville Health Care Center in Plainville, CT; Apple Rehab Colchester in Colchester, CT and Waterbury Extended Care Facility in Watertown, CT.
