A lack of communication between a discharging hospital and a residential-care facility is being blamed for the death of a 63-year-old man found drowned in a marsh, nine days after leaving the hospital.

Authorities were still searching at press time for relatives of Leverne Reed, whose body was found North Charleston, SC. They released his name as part of a public appeal to locate next of kin. He had had no family visitors or emergency contacts listed since arriving at the care facility in June 2014, the facility’s owner told police.

The owner, whose name was among the redactions in the police report, told authorities that once a hospital that has taken custody of a resident, it assumes responsibility of getting the resident back to the care facility. The care facility does not track the condition or whereabouts of residents who have gone to the hospital, he added.

Little is known of how Reed lived or died in his final days, according to a local report. The coroner said no foul play is suspected. The police file redacted the names of the hospital and residential facility.

According to the police report, a facility caretaker said Reed was taken by ambulance to the hospital on April 9 after suffering a seizure, but he was released the same day.
