Carol Johnston, 76, had been a resident of Crestwood Care Center in Shelby, OH, for around six weeks when she was found dead of a gunshot wound in her room on Saturday. Her husband, George, was found in the room injured with a self-inflicted gunshot wound. He was taken to the hospital, where he later died.

George Johnston, 77, had allegedly told employees of the nursing home that his insurance was running out for his wife’s stay in the facility, and that he felt he wouldn’t be able to care for her after her release, FOX 8 Cleveland reported. Authorities say George Johnston also left behind a note detailing his plans.

“We believe that this was a course of action agreed upon by both parties based on information that we have collected during the initial investigation,” authorities stated.
