Agitated transfer attempt, resident falls, $20,000 fine
The Illinois Department of Public Health has fined a facility $20,000 for an incident where an agitated resident fell from mechanical lift and hit her head, leading to her death. Straps on the lift slipped as the resident flailed at her aides, who were attempting to move her from her bed to a wheelchair.
The resident died Feb. 23, four days after incident, from a blood clot in the brain, according to state investigators. Aides at the 119-bed Apostolic Christian Home in Eureka said the resident came "combative" during the transfer and started hitting and scratching them, while also grabbing at the lift's straps as she was being moved. They said they tried to calm her and did not notice a strap had slipped.
In addition to the fine, the facility also was ordered to review its safety and transferring procedures.
