naviHealth Inc., a Cardinal Health company, has acquired RightCare Solutions Inc., a vendor providing hospital discharge planning software and readmissions management.

RightCare’s software was developed in conjunction with the University of Pennsylvania School of Nursing. It licenses its proprietary software to hospitals and health systems to assess patients for post-acute care needs. This includes determining the risk of readmission and coordinating patient discharges.

The company also licenses its software to post-acute care providers.

“RightCare has a proven track record of delivering technology solutions to hospitals that translate into real ROI. The combination of our post-acute decision support and clinical analytics with RightCare’s integrated discharge planning capabilities will further accelerate our ability to deliver a unique value proposition to health systems as they look to manage post-acute care in an evolving market,” said naviHealth CEO Clay Richards.

naviHealth now serves nearly 2 million health plan members and more than 75 hospitals.

RightCare was founded in 2012 and has its headquarters in the Philadelphia suburb of Horsham. RightCare co-founder Kathryn Bowles, Ph.D., was honored by the American Academy of Nursing last year.
