naviHealth and Golden Living announced a partnership last week to improve the patient care experience and reduce the cost of healthcare.

The partnership will use naviHealth’s “decision support technology, post-acute care analytics platform and network of clinicians to enhance patient outcomes and improve the transition of care for patients who desire to return home,” Golden Living said in a statement.

The companies said they will improve coordination of patient care and outcomes through a number of program elements, including the sharing of data, clinical collaboration on ER diversion and readmission programs, and inclusion of Golden Living providers in any proprietary provider networks established by naviHealth.
