Beth Israel Deaconess Medical announced it would use The Right Place, a digital health company, to help place patients in post-acute care facilities more efficiently.

BIDMC is using The Right Place patient placement and communication solution for discharges to any SNF in the country, including for more than 200 SNFs currently receiving referrals from its tertiary care center.

“We’re delighted to be one of over 200 care facilities now connected to The Right Place to receive and respond to electronic patient referrals from Beth Israel Deaconess Medical Center,” said Jason Twombly, director of admissions and marketing, Neville Center at Fresh Pond. “The Right Place web application opens up a direct line of communication to the case manager, which reduces turnaround time and makes the whole process more efficient. Since hospitals want to refer to facilities that can quickly handle electronic referrals, we expect The Right Place to not only make this process easier but also increase the number of referrals we receive.”

The company is HIPAA compliant and designed to let hospitals and nursing homes work together. It said it wants to improve the discharge process and let hospitals assess new Centers for Medicare & Medicaid Services quality measures, including the rate of rehospitalization, emergency room use, community discharge, and improvements in function.

The Right Place also launched a new web-based application that is free of charge to SNFs. It includes live chat functions and instant Medicare eligibility check.
