MDI Achieve, which provides MatrixCare™, announced it has partnered with Homecare Homebase.
Providers using MatrixCare™ and Homecare Homebase will gain benefits that include field connection to all patient/resident information, automated documentation of services provided, secure data exchange between long-term care facilities and homecare providers, improved continuity of care and ease of coordination, and simplified and consistent capture of patient/resident data. It’s an especially good solution for providers participating in accountable care organizations, according to MDI Achieve President John Damgaard.
Homecare Homebase integration will be completed using MatrixCare Connections, a program recently introduced by MDI Achieve.
