“Significant civil rights violations,” including the use of restraints, were found at a county-run nursing home in Alabama, the U.S. Justice Department announced Monday.

The agency said it conduced a four-month investigation before announcing its findings. Baxter County operates the 81-bed facility, Baxter Manor in Mountain Home, AR.

The probe “uncovered evidence of significant civil rights and statutory violations. Investigators found credible evidence of constitutionally deficient use of restraints and psychotropic medications and constitutionally inadequate rehabilitative and medical care.” The federal agency also believes Baxter Manor has not adequately assessed residents’ needs regarding continuing stays at the facility.

The Justice Department declared its findings, and recommendations, in a brief to Baxter County Judge Sue Newton, who will decide further action.
