The winners of the McKnight's Tech Awards will be announced this week.

By adopting a wireless daily living documentation tool, Hearthstone Health Center lowered its hospital readmission rates by 5.5%. That earned Hearthstone the Gold Award in the Transitions category of the McKnight’s Technology Awards — Connect Our World.

“Impressive results related to reductions in rehospitalizations pushed this application to the top,” one judge said of the Round Rock, TX-based long-term care facility. Hearthstone used AccuNurse’s ADL documentation tool, which allows nurses and aides to use the Inter Act II program and the SBAR tool.

The Transitions category recognizes excellence in technology used to improve and/or reduce transitions between long-term care settings and other providers.

Maryland-based Asbury Communities received the Silver Award in this category for its use of Vision electronic health records software, which was developed by HealthMEDX. Asbury is currently researching software that will use Vision data to identify residents at risk for adverse events.

West Park Long Term Care is taking home the Bronze Award for its integrated computer system and its hospitalist program. The program gives physicians at West Park’s critical access hospital and other area medical clinics access to the nursing home residents’ clinical information. The facility is located in northwestern Wyoming.

The Tech Awards are run in cooperation with American Health Tech.
