Alexandra (Alex) Berg, MSN, ARNP, a board-certified adult nurse practitioner, has been chosen as the director of transitional care for Able Care Connect. The Goodman Group, a seniors housing management company out of Minnesota, made the announcement.
Able Care Connect focuses on providing Transitional Care Management clinical services. Berg will work with older adults as they transition across healthcare settings. TCM is a reimbursed service approved by Medicare, and accepted by many other large insurance carriers for qualified participants.
Berg will be based at the Palms of Largo community which was, developed and managed by The Goodman Group.
Berg, who is a Largo, FL native, received her bachelor’s of science in nursing from Vanderbilt University and her master’s in nursing from the University of Virginia. She completed a nurse practitioner certificate program at Emory University. She has been a nurse practitioner since 2001.
